Inserting acupuncture needles superficially into various parts of the body can facilitate weight loss by giving the person a feeling of well-being which can suppress the desire for excess food. For these reasons, we do not recommend the use of ear staples for weight reduction.Diet and exercise are helpful in any program of weight reduction. Most people who come for acupuncture treatments, however, have been given diets and exercise regimens before. They may have good knowledge of what they should and shouldn't eat but they feel depressed or irritable when they try to stay on a diet. Acupuncture should relieve such problems and improve will power along with aiding in weight loss. The actual weight loss to be expected is about two to four pounds a week. Six to ten acupuncture treatments are usually sufficient, and these can be given once or twice a week. Weight loss should continue after the treatments are completed until normal weight is achieved. The effects of acupuncture usually last at least six months and sometimes a year or more. As early as in the Han dynasty, TCM practitioners recorded the symptoms, mechanism and risk factors of obesity. The Suwen (The Book of Plain Questions) states in chapter 28: "If obesity occurs in the nobleman and rich people, they must be over consuming heavy and greasy foods." Similar to the modern understanding of obesity, associated risk factors include undesirable eating habits, under exercising, body constitution and mental state. The ancient Chinese also knew obesity has close links with metabolic disorders such as diabetes and other conditions such as stroke or chest pain.The spleen is said to be at the root of all phlegm production. TCM does not treat "excess weight" per se; rather it deals with the underlying body condition that may have led to the excess weight. Once the body's balance is restored, the metabolism will process food properly and excess weight should no longer be a problem provided correct foods are eaten in moderate quantities. Specifically, TCM holds that fat or adipose tissue is mostly due to phlegm and dampness evils. The spleen is said to be at the root of all phlegm production and is involved in the transportation and transformation of body fluids and foods. If the spleen becomes damaged by eating too many sweet foods and getting too little exercise, it will fail in its duty to move and transform waste fluids and foods. Instead these metabolic wastes will gather, collect and transform into dampness evils. If dampness evils endure over time, they will congeal into phlegm, and become fat tissue.TCM takes a holistic approach to obesity by focussing on the underlying changes in the body. According to TCM principles, development of obesity is due to the following pathological changes.1. Dyspepsia causes stomach heat and poor spleen functioningOver consumption of heavy, greasy and spicy foods or alcohol facilitate production of heat evils in the stomach. Meanwhile, inadequate exercise after eating these types of foods damages the spleen function. The over-heated stomach will ripen an excessive amount of food.Therefore, the stomach will digest food easier and make an individual feel hungry, but the spleen cannot handle an excessive food load causing it to under function and be unable to carry out its transformation and transportation functions properly. High blood pressure - Additional fat tissue in the body needs oxygen and nutrients in order to live, which requires the blood vessels to circulate more blood to the fat tissue. This increases the workload of the heart because it must pump more blood through additional blood vessels. More circulating blood also means more pressure on the artery walls. Higher pressure on the artery walls increases the blood pressure. In addition, extra weight can raise the heart rate and reduce the body¡¯s ability to transport blood through the vessels. Diabetes - Obesity is the major cause of type 2 diabetes. This type of diabetes begins in adulthood. Obesity can cause resistance to insulin, the hormone that regulates blood sugar. When obesity causes insulin resistance, the blood sugar becomes elevated. Even moderate obesity dramatically increases the risk of diabetes. Heart disease - Atherosclerosis (hardening of the arteries) is present 10 times more often in obese people compared to those who are not obese. Coronary artery disease is also more prevalent because fatty deposits build up in arteries that supply the heart. Narrowed arteries and reduced blood flow to the heart can cause chest pain (angina) or a heart attack. Blood clots can also form in narrowed arteries and cause a stroke. Joint problems, including osteoarthritis - Obesity can affect the knees and hips because of the stress placed on the joints by extra weight. Joint replacement surgery, while commonly performed on damaged joints, may not be an advisable option for an obese person because the artificial joint has a higher risk of loosening and causing further damage. Sleep apnea and respiratory problems - Sleep apnea, which causes people to stop breathing for brief periods, interrupts sleep throughout the night and causes sleepiness during the day. It also causes heavy snoring. Respiratory problems associated with obesity occur when added weight of the chest wall squeezes the lungs and causes restricted breathing. Sleep apnea is also associated with high blood pressure. Cancer - In women, being overweight contributes to an increased risk for a variety of cancers including breast, colon, gallbladder, and uterus. Men who are overweight have a higher risk of colon and prostate cancers. Metabolic syndrome - Metabolic syndrome consists of six major components: abdominal obesity, elevated blood cholesterol, elevated blood pressure, insulin resistance with or without glucose intolerance, elevation of certain blood components that indicate inflammation, and elevation of certain clotting factors in the blood. In the US, approximately one-third of overweight or obese persons exhibit metabolic syndrome. Psychosocial effects - In a culture where often the ideal of physical attractiveness is to be overly thin, people who are overweight or obese frequently suffer disadvantages. Overweight and obese persons are often blamed for their condition and may be considered to be lazy or weak-willed. It is not uncommon for overweight or obese conditions to result in persons having lower incomes or having fewer or no romantic relationships. Type-1 and Type-2 Obesity Type-1 obesity is not caused by a disease and, in most cases, it is caused by excessive eating habits and lack of exercise. Type-2 obesity accounts for less than 1% of obesity cases and is caused by a disease; abnormal weight gains occur with type-2 obesity even when little is eaten. Cushing syndrome, hypothyroidism, polycystic ovarian disease, and insulinoma, are some internal secretion diseases that cause type-2 obesity. Cushing syndrome is characterized by thinning arms and legs and abdominal obesity, whereby fat gathers around the body, and in some instances, the hypodermis cracks from sudden weight gain causing line marks on the abdomen, groin, underarm, and back. Persons with hypothyroidism experience a drop in their metabolism, and weight gain occurs as a result of low calorie consumption. Type-1 obesity can be treated with general obesity treatment methods like simultaneous dietetic treatment and exercise. But type-2 obesity requires that the underlying disease be treated first. Usually, obesity goes away naturally after the underlying disease has been treated. Child-type and Adult-type Obesity Although obesity may look the same from the outside, a full examination reveals that there's a considerable amount of personal difference in the number and size of fat cells. According to the size and number of fat cells, obesity can be divided into adult-type, where only the size of fat cells is increased, and child-type, where the number of fat cells is increased. The number of fat cells increases the most during the year after birth. People who have always been obese since childhood, have 3-4 times more fat cells than people who became obese as adults. With adult-type obesity, the number of fat cells is close to normal, but the size increases. This happens mostly after middle age. But generally, the reason child-type obesity is more dangerous and important than adult-type obesity is because it is extremely difficult to reduce the number of fat cells already made. Therefore, treatment is much more effective for adult-type obesity, where the enlarged fat cells just have to be returned to their original state, than for child-type obesity. Abdominal Obesity and Limb Obesity Depending on where fat is stored on the body, obesity can be divided in to abdominal obesity and limb obesity. Abdominal obesity is when fat is mostly distributed over the abdomen and back, and limb obesity is when fat is concentrated around the thighs, arms, legs, and buttocks. Abdominal obesity occurs mainly in men; limb obesity occurs primarily in women. The reason the distribution of fat is significant is because depending on where it's concentrated the danger level for adult diseases changes. So with abdominal obesity there's a higher danger of ischemic heart disease, diabetes, and hyperlipidemia. This is because low protein steatolytic enzymes, which breakdown nutrients into triglycerides and store them in fat cells, are very active within abdominal fat. Young people generally become overweight or obese because they do not get enough physical activity in combination with poor eating habits. Genetics and lifestyle also contribute to a child's weight status.Recommendations for prevention of overweight and obesity during childhood and adolescence include:1,Gradually work to change family eating habits and activity levels rather than focusing on a child's weight. 2,Be a role model. Parents who eat healthy foods and participate in physical activity set an example so that a child is more likely to do the same. 3,Encourage physical activity. Children should have 60 minutes of moderate physical activity most days of the week. Obesity refers to being more than 20% over your desirable weight. Unfortunately, it is all too common in the developed world where our modern lifestyle tends to make us eat more and move less.
Obesity and its associated disorders are leading causes of morbidity and premature mortality around the world. Obese persons are also vulnerable to low self-esteem and depression because of the psychological and social stigmata that can be associated with obesity. Despite societal prejudicial perceptions that obesity develops because of deficient self-control, research has provided insight into the physiology behind unwanted weight gain. Indeed, during the past decade, the field of body-weight regulation (the study of the homeostatic mechanisms controlling body weight and fat content and the pathophysiology leading to unwanted weight gain or weight loss) has undergone an explosion in research, particularly in the area of neuroendocrine control of appetite and energy expenditure. As with other leading diseases in developed countries, such as hypertension and diabetes, obesity is recognized as a chronic condition resulting from an interaction between environmental influences and an individual's genetic predisposition to weight gain.
The initial evaluation of overweight and obese patients begins with the exclusion of secondary causes of weight gain and the identification of comorbid disorders such as hypertension, diabetes, heart disease, and sleep apnea. Once screening is completed, the approach to the treatment of overweight and obesity is similar to that of other chronic diseases: begin with lifestyle improvements, and then consider medical and surgical options. Although the weight loss that accompanies current therapeutic options is modest on average, the future promises better diagnostic and treatment options for obesity that are based on research into the mechanisms of weight regulation and their role in unwanted weight gain and maintenance of the obese state.
